(A) IN RESPONSE TO A REQUEST TO PROVIDE OR VERIFY PAST OR PRESENT EMPLOYMENT.
(B) IN RESPONSE TO A REQUEST BY A PROPERLY IDENTIFIED LAW ENFORCEMENT AUTHORITY.
(C) TO PROTECT THE LEGAL INTEREST OF THE COMPANY.
(D) TO PREVENT DAMAGE TO THE PROPERTY OF THE COMPANY OR PHYSICAL INJURY TO ANY OF THE COMPANY'S EMPLOYEES.
(E) PURSUANT TO A COMPULSORY STATUTE OR REGULATION REGARDING SUCH A DISCLOSURE.
(F) PURSUANT TO A COLLECTIVE BARGAINING AGREEMENT PROVIDING FOR DISCLOSURE TO MY BARGAINING REPRESENTATIVE.
(G) TO AN AGENT OR CONTRACTOR OF THE COMPANY PROVIDED THAT ONLY INFORMATION THAT IS NECESSARY FOR THE AGENT OR CONTRACTOR TO PERFORM ITS FUNCTION SHALL BE DISCLOSED AND THE AGENT OR CONTRACTOR SHALL BE PROHIBITED FROM FURTHER DISCLOSURE OF SUCH INFORMATION.
(H) IN RESPONSE TO A LAWFULLY ISSUED SUMMONS, JUDICIAL ORDER, SEARCH WARRANT, OR SUBPOENA.
THIS AUTHORIZATION EXPIRES ONE YEAR FROM DATE OF SIGNING. (IF WE HAVE NOT CONTACTED YOU ABOUT EMPLOYMENT WITHIN ONE YEAR FROM THE DATE OF THIS APPLICATION, YOU MUST RETURN TO THIS OFFICE OR CONTACT US BY PHONE TO INDICATE YOUR CONTINUED INTEREST IN ORDER TO RECEIVE FURTHER CONSIDERATION FOR EMPLOYMENT.)
OVEC is a direct deposit pay company. If hired, I agree to direct deposit payments and will provide the Company with personal account information, as required, in order to make electronic deposits at a financial institution equipped to accept such deposits.
I hereby voluntarily authorize Ohio Valley Electric Corporation/Indiana-Kentucky Electric Corporation (hereinafter the “Company”) to conduct an inquiry into my work experience, education, general reputation and character. This inquiry may be conducted either by the Company through its employees, or by an independent agency retained by the Company through its employees, or by an independent agency retained by the Company, and may include but is not limited to conviction records, motor vehicle records, credit checks, references, drug test results, and copies of prior personnel files. I authorize and request any and all of my former employers, personal references, education institutions, law enforcement and credit agencies, and/or any other person to furnish to the Company or its agents, any information that they may have concerning the above-referenced matters. This may include, but is not limited to, educational history, a history of my employment and the reasons for termination thereof, criminal information, motor vehicle records, drug test results and credit records.
I hereby release the Company, or any agency retained by the Company, from all liability and responsibility, in connection with making the aforesaid inquiry, including their agents and employees. Further, I hereby release from any and all liability whatsoever all persons, institutions, employers, organizations and/or agencies providing information pursuant to this authorization and request.
